Fees: $250 initial education fee, plus additional charges (see below for details)
These sessions will be instructed by a CDM faculty member in a one-to-one clinical setting. Prior to performing the implant placement, all Part II participants must first observe two implant procedures in the CDM clinic. Participants may also opt for observation only. Additional course fees will apply.
You must provide the following:
1) copy of your dental license
2) Short bio and/or cv
3) Signed health care paperwork indicating all necessary professional requirements are up-to-date; includinig protection from hepatitis and tb
4) Signed patient privacy form
5) Your dental certificate of insurance with Columbia University and its employees named as additionally insured
Part II a) Clinical Observations: An educational fee of $250 is required to cover observation of two implant procedures in the Implant Suite, Vanderbilt Clinic. Payable by check to: Columbia University.
You may choose to participate only in part a (observation) or continue to part b (implant placement), upon approval and working up a case with one of the course directors.
Part II b) Implant Placement: Various options are available for your convenience, based on the needs and comforts of the patient*:
Education Fee: When you work with and provide your own patient in the CDM clinic, a $1,250 education fee applies, payable by credit card or check.
Once the patient is approved-- the case has been worked up with one of our course directors with all necessary paperwork completed--surgery is scheduled. Depending on the case, you may have the option of placing up to two implants. After surgery you will be responsible for doing the restoration work in your own office. All patients MUST be approved by the course director.
Materials Fee: You may provide your patient's cone beam scans. Otherwise a fee of $150 per scan will be charged by the clinic. Implants ($500 each); cone bean scans ($150 each) and other materials fees, the cost of which may be covered by you or your patient on the day of surgery prior to implant placement.
- When you identify a patient requiring more complex work, such as a sinus lift, you assist a Columbia Faculty member or resident doing the surgery. Pricing depends on the needs of the patient, the cost of which can be covered by your or your patient. In addition to the cost of the cone beam scan and implant, a $1,000 fee applies for sinus liftsand a $1,000 fee applies for advanced bone graft.
Your patient will need to register as a Columbia patient. They do this directly with the clinic upon arrival at VC9 checkin desk. Be sure to arrive at least a ½ hour before your appointment to ensure this process runs smoothly.

LOCATION Courses are held at Columbia University Medical Center on West 168th Street unless otherwise listed. Room details will be sent to confirmed registrants. Parking is not provided for most courses.
COURSE STRUCTURE & REQUIREMENTS Courses are lecture and hands-on, model-based format and are open to general and specialty practitioners.

Registration may also be done by telephone at 212-305-7124. Please make checks payable to: Columbia University. Early registration is advised to guarantee a place in all courses.
TUITION & FEES A deposit is required to secure enrollment. Tuition must be paid in full prior to start of course. CDM alumni and all dental hygienists/staff receive a 10% discount on listed tuition for all continuing education courses. Alumni graduating within five (5) years prior to a course receive a 20% discount. CDM faculty & staff receive a 50% discount on listed tuition rates.
CHANGES & CANCELLATIONS Refunds are granted only if notification is received at least 3 weeks prior to start of course. Cancellation after this time will result in a $150 processing fee. Columbia University cannot assume responsibility for losses due to participants' travel arrangements. Tuitions for no-shows will be forfeited.
